Who Uses Report: Claims management to determine amounts paid, per claim number, to payees having tax id numbers.

When to Run Report: Can be run any time.

Input Parameters:

• Start Date: MM/DD/YYYY

• End Date: MM/DD/YYYY

• Company / State / Line of Business (With a selection criteria including "All" companies / states / lines of business)

• Individual agency or an option to include "All" agencies
